About The Position

The Occupational Therapist (OT) at Port Lavaca Nursing and Rehabilitation Center is responsible for assessing and reassessing patients' rehabilitation needs and potential. This role involves planning, implementing, and supervising therapeutic programs focused on improving or restoring physical and sensory-integrative functions, and enhancing independence in daily living activities. The OT will apply diagnostic and prognostic procedures, collaborate with the Rehab Director, provide patient education, and document patient progress and involvement in their care. Additionally, the position requires instructing family members and other personnel, participating in case conferences, and contributing to the agency's continuous quality assessment and improvement initiatives.

Requirements

  • Graduate of a curriculum in Occupational Therapy accredited jointly by the committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association awarding a Bachelor’s degree or higher.
  • Current licensed by the state of Texas to practice as a Occupational Therapist.
  • Two years appropriate experience as an Occupational Therapist required.
  • Must have a car with current insurance coverage and a State driver’s license.
  • Ability to work a minimum of eight (8) hours per day while driving, standing, sitting, assisting patients movement, writing, and speaking.

Nice To Haves

  • Two-year experience as an Occupational Therapist in home health preferred.

Responsibilities

  • Conducts assessments and reassessments of rehabilitation needs and potential.
  • Conducts plans, implements, and supervises therapeutic programs which include patient teaching and therapeutic activities to improve or restore physical functions, sensory-integrative functions, and greater independence in activities of daily living.
  • Conducts assessments and reassessments of the patient’s level of function by applying diagnostic and prognostic procedures.
  • Plans, implements, and supervises an individualized therapeutic program designed to restore physical function, sensory-integrative functions, or greater independence in activities of daily living.
  • Coordinates and collaborates with the Rehab Director, or designed professional, to ensure integration of the therapeutic program in the plan of care.
  • Guides and teaches the patient in his/her use of therapeutic, compensatory, creative, and self-care activities for the purpose of improving function and increasing independence in activities of daily living.
  • Observes, records, and reports the patient’s reaction to treatment and any changes in the patient’s condition or treatment.
  • Documents the extent of the patient’s involvement in the decision affecting his/her care.
  • Instructs other agency personnel and patients family members in certain phases of occupational therapy in which they may safely and effectively assist the patient.
  • Participates in patient case conferences to coordinate the patient’s plan of care and promote positive therapeutic outcome.
  • Participates in continuous assessment and evaluation of the agency via Quarterly Record Review, or the quality Assurance/ Improvement Program.
  • Completes work, care and documentation with accuracy within agency time frames.
  • Maintains agency property, supplies and equipment in a manner that demonstrates ownership and accountability.
  • Maintains the work area to reduce the likelihood for safety hazards and to enhance its general appearance.
  • Attends and participates positively in meetings.
  • Adheres to agency infection control and safety polices, including education, reporting, and practice implementation specific to job position.
  • Attends agency provided in-service programs to fulfill requirements of position and agency polices.
  • Contributes to teamwork, and harmonious working relationships.
  • Supports and demonstrates continuous improvement of quality and service.
  • Participates in problem solving and reports problems, unusual events appropriately.
  • Uses own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
  • Continuously analyze work process and makes suggestions for improvement.
