About The Position
UPLIFT HOMECARE, LLC provides professional, affordable, and compassionate in-home living assistance for Seniors and Veterans in the local area. Our services include caring companionship, personal care, medication reminders, and support with daily activities to enhance quality of life. We are seeking a dedicated and experienced Occupational Therapist (OT) to join our team in Brandon, FL. This is an immediate opening for a motivated professional ready to make a meaningful impact in patients’ lives through high-quality home health care.
Requirements
- Graduate of an accredited Occupational Therapy Program and active OT license in Florida
- CPR certification and AHCA level 2
- Knowledge of state and federal healthcare laws and regulations
- Good clinical judgment, strong interpersonal skills, resourceful and compassionate
- Minimum 1 year of home health experience required
- Strong clinical and communication skills
- Ability to work independently and manage time effectively
- Reliable transportation
Responsibilities
- Evaluate and assess patients in their homes to develop individualized treatment plans
- Implement therapeutic interventions to improve patients’ daily living and functional abilities
- Educate patients and caregivers on treatment plans and home exercises
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely documentation
