Requirements

  • Active Minnesota Occupational Therapist (OTR/L) license.
  • Strong clinical reasoning and patient-centered approach.
  • Excellent communication and organizational skills.
  • Ability to work independently while managing a field-based schedule.
  • Valid driver's license and reliable transportation.

Nice To Haves

  • Home health experience is welcomed but not required.

Responsibilities

  • Complete comprehensive occupational therapy evaluations in patients' homes.
  • Develop individualized treatment plans focused on ADLs, safety, upper extremity function, cognition, and independence.
  • Recommend adaptive equipment, home modifications, and energy conservation strategies.
  • Educate patients and caregivers to promote long-term success and reduce fall risk.
  • Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team.
  • Maintain accurate and timely documentation using our electronic medical record.
