Requirements
- Active Minnesota Occupational Therapist (OTR/L) license.
- Strong clinical reasoning and patient-centered approach.
- Excellent communication and organizational skills.
- Ability to work independently while managing a field-based schedule.
- Valid driver's license and reliable transportation.
Nice To Haves
- Home health experience is welcomed but not required.
Responsibilities
- Complete comprehensive occupational therapy evaluations in patients' homes.
- Develop individualized treatment plans focused on ADLs, safety, upper extremity function, cognition, and independence.
- Recommend adaptive equipment, home modifications, and energy conservation strategies.
- Educate patients and caregivers to promote long-term success and reduce fall risk.
- Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team.
- Maintain accurate and timely documentation using our electronic medical record.
