About The Position

Baptist Health Home Care is looking for an Occupational Therapist (OT) to join their team. This role focuses on providing home-based therapy care for patients as directed by the attending physician. The OT will work as part of an interdisciplinary team, focusing on compassionate quality care and positive patient outcomes. The position involves interacting with patients' families and supporting patients throughout their recovery journey, utilizing the home environment to provide context and real-world perspective for restoring health.

Requirements

  • Occupational Therapist (OT) with a current license in the state of employment.
  • Minimum one-year experience as an Occupational Therapist (OT) in an acute care setting.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
  • Compassionate communicator with a positive attitude.
  • Patience is a virtue when working with patients, families, physicians, and coworkers.
  • Attention to detail is critical, as is being observant and following directions.

Nice To Haves

  • Home care experience preferred.

Responsibilities

  • Provide skilled occupational therapy as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
  • Develop an individualized OT plan of care and make adjustments as needs change.
  • Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.
  • Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.
  • Teach the patient on preventative measures and help them make good decisions to decrease re-hospitalizations.
  • Counsel patient and family members on the disease/injury and how to manage in all phases of occupational therapy.
  • Instruct patient and family members on the proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Oversee and supervise total care of patient provided by COTA.
  • Communicate with the COTA on supervisory visit results.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Understand and follow agency policies, procedures, rules, and regulations.
  • Communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
  • Educate patients and their family members and ensures the safety of the patient.
  • Complete accurate documentation and remain compliant with all legal rules and regulations.
