About The Position

We are seeking a PRN Occupational Therapist to join our dynamic team! Position starts at $75/visit! Our care management program has been in existence for over 25 years and was originally part of the Lee Memorial Health System.

Requirements

  • Valid and current Florida Occupational Therapist License
  • One (1) year relevant experience in occupational therapy

Responsibilities

  • Assists the physician in evaluating level of function, helps develop the plan of care (revising as necessary), advises and consults with the family and other agency personnel.
  • Performs comprehensive assessment utilizing the OASIS data set.
  • Provides occupational therapy services as prescribed by a physician, which can be safely provided in the home and to assist the physician in evaluating the patient's level of function by applying diagnostic and therapeutic procedures.
  • Provide treatment to patients per Cypress at Home protocols, utilizing Wellsky (Kinnser) assessment and documentation.
  • Guides the patient in the use of therapeutic, creative and self-care activities for the purpose of improving function.
  • Observes and records activities and findings in the clinical and progress notes and records, and reports to the physician the patient's reaction to treatment, changes in condition, or when there are deviations from the plan of care.
  • Instructs the patient, caregivers and other health team personnel, when appropriate, in therapeutic procedures of occupational therapy.
  • Maintains confidentiality of all patient, agency and employee information.
  • Participates in training and in-service programs as requested.
  • Participates in Agency PI activities as requested.
  • Provides direct supervision to Occupational Therapy Assistants as per requirement.
  • Attains quarterly goals as established in the annual performance evaluation.
  • Submits required documentation in a timely manner.
