About The Position

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time opportunity performing outpatient home health visits. This role involves serving adult and geriatric patients in their homes within Addison County, Vermont, as part of a multi-disciplinary health care team focused on rehabilitative care and skilled nursing to enable clients to remain at home. The position offers flexible schedules with no holidays or weekends required, and a bonus is available for full-time employees.

Requirements

A current Vermont Occupational Therapist license.
A minimum of one year of recent work experience as an Occupational Therapist.
Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal skills, organizational skills, and problem-solving skills.
Solid computer skills.

Nice To Haves

Prior home care experience preferred.
Prior experience with electronic medical records (EMR) preferred.

Responsibilities

Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group homes for adults with intellectual disabilities.
Operate professionally at the top of licensure, utilizing current research-based approaches to maximize functional independence for clients.
Collaborate with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment.
Educate clients, families, and community caregivers on care plans to foster a team approach for maximizing functional independence.
Perform evaluations, treatments, and discharge planning using an outpatient EMR platform to document individualized treatment approaches.
