Health Sciences North (HSN) is seeking an innovative individual for the position of Occupational Therapist to provide a holistic, client, and family-centred approach to assessment, analysis and treatment to optimize safety, functional ability, and well-being to enable participation in activities of daily living for older adults with multiple complex health needs. Provide care to patients in acute care transitioning to the Acute and Reactivation Care Centre. The Acute and Reactivation Care Unit (ARCC) is a 52-bed unit that offers a model of geriatric inpatient acute care that integrates the geriatric standard of care, Alternate Level of Care (ALC) leading practices, and rehabilitative care best practices for older adults living with/at risk of frailty. The ARCC has been intentionally designed to support achievement of hospital-wide patient flow by ensuring that patients - older adults with complexity and/or frailty – are placed on the appropriate clinical unit with the clinical team that has disease- or condition-specific expertise. As a result, an evidence-based restorative care pathway for hospitalized older adults has been implemented at HSN that optimizes function while treating acute needs. The ARCC model of care includes higher medical acuity, where at-risk older adults are admitted directly to the ARCC from the Emergency Department (ED) with a primary goal of medical stabilization and concurrent focus on functional optimization. It also includes transition prior to ALC designation, where at-risk older adults are proactively identified and transitioned to ARCC from the ED and other HSN acute care units prior to ALC designation.
