The Occupational Therapist is responsible and accountable for residents’ evaluation, ongoing assessment, planning, and implementation of therapy interventions. This role involves developing and revising the plan of care based on residents' needs, establishing functional goals, outcomes, and discharge plans. The therapist will also plan for residents' discharge, anticipating both long-term and short-term needs, and educate other healthcare team members, residents, family, and caregivers on the care plan.
