About The Position

Human Touch Home Health is seeking a Full-Time Occupational Therapist (OT) to join our growing Fredericksburg-area team. Patient opportunities may include Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and surrounding communities. We work to build manageable routes and patient assignments based on location and service needs. Requirements

Active Virginia Occupational Therapist License Required
Graduate of an accredited Occupational Therapy program
Valid Driver's License and reliable transportation
Strong clinical assessment and communication skills
Ability to manage a full-time home health caseload
Ability to work independently in patients' homes

Nice To Haves

Home Health Experience Preferred, but Not Required
WellSky Experience is a Plus

Responsibilities

Complete occupational therapy evaluations in patients' homes
Develop individualized plans of care
Help patients regain independence with activities of daily living
Provide therapeutic interventions to improve strength, coordination, mobility, and function
Assess home safety and recommend adaptive equipment or modifications
Educate patients and caregivers
Monitor progress and adjust treatment plans as needed
Coordinate care with physicians, nurses, therapists, and other members of the care team
Complete accurate and timely documentation using WellSky
Participate in discharge planning
