This is an on-call/per diem position supporting all western WA PACE sites. The Occupational Therapist (OT) is responsible for OT evaluations and the development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and is responsible for communicating and coordinating care with the team, as well as multiple outside providers, caregivers, and family members. The OT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assisted Living Facilities, and nursing homes. The OT assumes responsibility for participants treated by the OTA and OT Aide.
