About The Position

The Casual Home Care Occupational Therapist will enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the service area of our patients is the 11 counties of the Twin Cities Metro Area. Interim HealthCare offers a wide variety of home care ranging from skilled nursing, physical therapy and other licensed health care services, which help with the every day tasks that let people continue living at home where they are most comfortable. Additionally, they provide hospice care in the home that can make loved ones as comfortable as possible and provide support to the family, allowing them to concentrate on spending quality time together. Interim’s mission is firmly rooted in its long tradition of helping people maintain their safety, independence and dignity in an environment of mutual respect and compassion.

Requirements

Current license in the State of Minnesota as an Occupational Therapist (OT).
Two (2) years of experience as an Occupational Therapist.
Able to travel within geographic area serviced by the office.
Valid driver's license, reliable transportation, and proof of auto insurance required.
CPR Certification

Responsibilities

Completes initial and ongoing comprehensive assessments at appropriate time points as assigned.
Participates in ongoing interdisciplinary assessment of the patient.
Provides services that are ordered by the physician as indicated in the plan of care.
Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
