About The Position

Interim HealthCare is hiring a full-time Home Health Occupational Therapist to serve patients in Merrillville, Indiana, and surrounding areas. The therapist provides personalized, home-based care to help patients regain independence and perform everyday activities. The role works with patients in their homes and as part of a home health team focused on each patient’s plan of care and personal goals. The listed salary range is $80,000–$95,000 per year, plus mileage reimbursement and bonus opportunities each pay period. Requirements

Graduate of an accredited Occupational Therapy Program.
Active occupational therapy license in Indiana.
At least 1 year of occupational therapy experience; home healthcare experience is identified as ideal.
CPR certification.
Knowledge of state and federal home health regulations.
Good clinical judgment, strong interpersonal skills, resourcefulness, and compassion.

Responsibilities

Provide occupational therapy to patients who cannot perform daily tasks because of illness or injury.
Work with a home health team that may include an RN, LPN, CNA, HHA, PT, and SLP, following the patient’s plan of care and personal goals.
Assess patients, observe deficits, establish therapy goals, and document progress.
Help patients perform exercises to improve fine motor skills and coordination.
Recommend adaptive equipment, such as grab bars and shower chairs, to provide added support.
Assess fall risks and introduce strategies to improve home safety.
Educate patients and families about the plan of care, exercises, goals, and self-care.
