Rossmoor Post Acute is seeking a passionate and motivated Occupational Therapist (OTR) to join our premier in-house rehabilitation team in beautiful Walnut Creek, California. Requirements
- Current California Occupational Therapist (OT) license in good standing
- Strong communication and teamwork skills
- Passion for functional rehabilitation and resident-centered care

Responsibilities
- Develop and implement individualized occupational therapy treatment plans
- Help residents improve independence with Activities of Daily Living (ADLs)
- Supervise and support Certified Occupational Therapy Assistants (COTAs)
- Collaborate with nursing and interdisciplinary teams to optimize resident care
- Participate in Patient Care Conferences and Utilization Review meetings
- Maintain accurate and timely documentation in compliance with state regulations
- Provide caregiver and family education for safe discharge planning
- Communicate with physicians regarding treatment updates and care recommendations
