About The Position

The Home Care Occupational Therapist delivers skilled therapy services to patients in their homes to help them regain, maintain, or improve independence in daily living activities. This role involves evaluating functional abilities, creating individualized treatment plans, and providing hands-on interventions to improve safety, mobility, and quality of life in a home setting.

Requirements

  • Degree in Occupational Therapy from an accredited program (BS, MS, or Doctorate)
  • Current Occupational Therapist license in the state of practice
  • NBCOT certification (if required by state)
  • Valid driver’s license and reliable transportation
  • Ability to travel to patient homes within assigned territory
  • Ability to work independently in a home-based environment
  • Strong clinical judgment and problem-solving skills
  • Excellent communication and patient education abilities
  • Compassionate and patient-centered approach to care
  • Ability to safely assist patients with mobility and functional tasks

Nice To Haves

  • Prior experience in home health or community-based therapy
  • Experience working with older adults or medically complex patients
  • Familiarity with EMR documentation systems
  • Strong organizational and time management skills

Responsibilities

  • Conduct in-home occupational therapy evaluations to determine patient needs and functional limitations
  • Develop and implement individualized care plans focused on improving independence with daily activities
  • Provide skilled therapy interventions to address physical, cognitive, and functional deficits
  • Assess home environments for safety risks and recommend appropriate modifications
  • Train patients and caregivers on adaptive strategies, energy conservation, and safe mobility techniques
  • Recommend and educate patients on adaptive equipment and assistive devices
  • Document patient evaluations, treatments, progress, and discharge summaries in a timely and accurate manner
  • Collaborate with physicians, nurses, physical therapists, and other care team members to coordinate patient care
  • Monitor patient progress and adjust treatment plans based on clinical outcomes
  • Educate patients and families on injury prevention, condition management, and functional maintenance
  • Follow all regulatory requirements, agency policies, and reimbursement guidelines
