About The Position

Stern at Home Therapy is hiring an Occupational Therapist to provide one-on-one home health care to a primarily geriatric patient population throughout Bergen County, New Jersey. The role offers a choice between part-time W-2 employment and flexible PRN work. Therapists can build a regular weekly caseload or accept visits around their existing schedule, with control over availability and preferred coverage area. Care is delivered one patient at a time, without double-booking or productivity quotas, and therapists have clinical autonomy over evaluations and individualized treatment plans. Dedicated administrative support is available for scheduling, coordination, and patient logistics.

Compensation is per visit. Part-time W-2 rates are $65 per follow-up visit and $75 per four-unit evaluation. PRN rates are $72 per four-unit follow-up visit and $85 per evaluation. Eligible part-time W-2 employees may receive a medical wellness stipend, quarterly wellness stipend, 401(k) with annual match, student loan repayment program, CEU support, and provided equipment. Shadowing is available for occupational therapists new to home health.

Requirements

• Active Occupational Therapist license in New Jersey.
• Reliable transportation.
• Strong clinical, communication, and time-management skills.
• Ability to work independently in a home-based setting.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient's functional needs and goals.
• Manage individualized treatment plans with clinical autonomy.
• Help patients improve independence, safety, mobility, and ability to perform daily activities.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, and members of the clinical team.
• Coordinate with the administrative team regarding scheduling, patient care, and logistics.
• Build and manage a caseload based on availability and preferred coverage area within Bergen County.
