Human Touch Home Health is seeking a compassionate Occupational Therapist (OT) for PRN home health opportunities throughout Loudoun County, Virginia. We have patient opportunities in Leesburg, Ashburn, Sterling, Purcellville, South Riding, Aldie, Middleburg, Hamilton, Lovettsville, and surrounding Loudoun County communities. You do NOT have to cover the entire county. Choose the cities and areas that work best for you and build your caseload around your availability. Requirements

Active Virginia Occupational Therapist License Required
Graduate of an accredited Occupational Therapy program
Valid Driver's License and reliable transportation
Strong clinical assessment and communication skills
Ability to work independently in patients' homes

Nice To Haves

Home Health Experience Preferred, but Not Required
WellSky Experience is a Plus

Responsibilities

Complete occupational therapy evaluations in patients' homes
Develop individualized plans of care
Help patients improve independence with activities of daily living
Provide therapeutic interventions to improve strength, coordination, mobility, and function
Assess home safety and recommend adaptive equipment or modifications
Educate patients and caregivers on safe techniques and strategies
Monitor progress and adjust treatment plans as needed
Coordinate care with physicians, nurses, therapists, and other members of the care team
Complete accurate and timely documentation using WellSky
Participate in reassessments and discharge planning
