About The Position

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time, outpatient home health position. This role involves performing home visits for adult and geriatric patients in independent and assisted living communities. The OT will be part of a multi-disciplinary team providing rehabilitative care to help clients remain at home. The position is based out of the Wilmington Senior Living Office and services the North Wilmington/Hockessin area.

Requirements

A current DE Occupational Therapist license.
A minimum of one year of recent work experience as an Occupational Therapist.
Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
Ability to work independently and manage time effectively.
Strong interpersonal skills, organizational skills, and problem-solving skills.
Solid computer skills.

Nice To Haves

Prior home care experience preferred.
Prior experience with electronic medical records (EMR) preferred.

Responsibilities

Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group homes for adults with intellectual disabilities.
Utilize current research-based approaches to maximize functional independence for clients.
Collaborate with home health therapy partners to transition care from home health to outpatient services.
Educate clients, families, and community caregivers on care plans to foster a team approach.
Perform evaluations, treatments, and discharge planning using the outpatient EMR platform to document individualized treatment approaches.
