About The Position

FellowshipLIFE is seeking a dedicated and compassionate Occupational Therapist (OT) to join our Rehabilitation team on a per diem basis. In this role, you will help residents regain independence in activities of daily living following illness, injury, surgery, or changes in functional status. Working closely with residents, families, and the interdisciplinary care team, you will play a vital role in maximizing function, promoting independence, and enhancing quality of life. Requirements

  • Graduate of an accredited Occupational Therapy program.
  • Current New Jersey Occupational Therapist license required.
  • Strong clinical assessment, treatment planning, and intervention skills.
  • Excellent interpersonal, verbal, and written communication skills.
  • Ability to work effectively within a multidisciplinary healthcare team.
  • Commitment to providing compassionate, high-quality care to older adults.

Responsibilities

  • Evaluate residents and develop individualized occupational therapy treatment plans based on clinical assessments and resident goals.
  • Provide occupational therapy treatments designed to improve functional abilities related to self-care, home management, mobility, and activities of daily living.
  • Develop and implement individualized plans of care to maximize resident independence and safety.
  • Educate residents, families, and caregivers on adaptive techniques, home safety, and the use of assistive devices and adaptive equipment.
  • Collaborate with Physical Therapists, Speech-Language Pathologists, nursing staff, physicians, and caregivers to coordinate comprehensive resident care.
  • Monitor resident progress and modify treatment plans as appropriate to achieve optimal outcomes.
  • Maintain accurate and timely clinical documentation in compliance with facility policies and regulatory requirements.
  • Participate in interdisciplinary care planning meetings and rehabilitation team discussions.
  • Assist with group therapy programming when appropriate.
  • Ensure resident safety and independence during all therapy interventions.
