About The Position

Signature Bend Home Health is seeking a compassionate and motivated PRN Occupational Therapist to join our growing team. In this role, you will provide skilled therapy services in patients' homes, helping them regain independence, improve daily functioning, and achieve their highest level of well-being. Requirements

Current Occupational Therapist license in the state of practice (if required)
Graduate of an ACOTE-accredited Occupational Therapy program
Eligible for or certified through NBCOT
Current BLS/CPR certification
Valid driver's license, reliable transportation, and current auto insurance

Nice To Haves

Home health or clinical experience preferred (2+ years preferred)
Strong communication, documentation, and organizational skills

Responsibilities

Evaluate patients and develop individualized treatment plans.
Provide occupational therapy services as ordered by the physician and outlined in the plan of care.
Assess functional abilities, home safety, and adaptive equipment needs.
Help patients improve daily living skills, mobility, and independence.
Educate patients, families, and caregivers on treatment goals and home programs.
Collaborate with physicians and the interdisciplinary care team to ensure quality patient outcomes.
Maintain accurate and timely clinical documentation.
Participate in care coordination, quality improvement activities, and discharge planning.
Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state and organizational guidelines.
Train patients in the use of adaptive equipment, splints, prosthetics, and orthotics as appropriate.
