About The Position

Lakes Region VNA and Hospice is seeking a dedicated Occupational Therapist to provide high-quality therapy services that help patients regain independence and improve their quality of life. The therapist evaluates patient needs, develops treatment plans, and collaborates with healthcare professionals to support comprehensive care. This is a full-time traveling position serving Barnstead, Laconia, and surrounding areas. Reliable transportation is required for travel in varied weather conditions.

Requirements

Graduate of an accredited Occupational Therapy program.
Licensed with the NH State Board.
Minimum of 2 years of clinical experience as an Occupational Therapist.
Current CPR certification.
Valid driver’s license and reliable, insured vehicle.
Strong verbal, written, and organizational skills.
Ability to walk or stand for prolonged periods.
Ability to lift, position, or transfer patients up to 50 lbs.
Frequent reaching, stooping, bending, kneeling, or crouching.
Adequate vision and hearing to perform required duties.

Responsibilities

Evaluate patients’ functional status and home environment for hazards or barriers to independence.
Develop individualized treatment programs and establish goals for improved function.
Teach new skills or retrain patients in daily activities lost due to illness or injury.
Fabricate splints and instruct patients in the use of adaptive equipment, prosthetics, and orthotics.
Maintain accurate clinical records and communicate changes in patient condition to physicians.
Participate in care planning, discharge planning, and interdisciplinary meetings.
Supervise Certified Occupational Therapy Assistants and home health aides as required.
Ensure compliance with organizational policies and state regulations.
