About The Position

H2 Health Prime Living is seeking an Occupational Therapist to provide 1:1 patient care within senior living communities in the Beaches area of Jacksonville, FL. The role focuses on helping seniors maintain independence, confidence, and quality of life through meaningful, goal-driven interventions. The Occupational Therapist will also collaborate with home health partners within the same communities to ensure care continuity and seamless transitions.

Requirements

• A graduate of an accredited Occupational therapy program.
• An active or eligible state license to practice.

• Prior experience is preferred, but new graduates are welcome!
