About The Position
Ultimate Therapy is hiring an Occupational Therapist (OT) to join our rehabilitation department within a skilled nursing facility. We are seeking a therapist who enjoys helping residents regain independence, improve functional abilities, and return to meaningful daily routines through individualized care plans. Birch Manor at Autumn Lake offers a collaborative rehab setting with strong interdisciplinary teamwork and the opportunity to work with a diverse geriatric and post-acute population. We provide a stable caseload with meaningful resident relationships and a leadership team focused on therapist support, mentorship, and growth.
Requirements
- Active Occupational Therapist license required
- Strong communication and clinical reasoning skills
Nice To Haves
- Skilled nursing experience preferred but not required
Responsibilities
- Conduct occupational therapy evaluations and establish resident-centered treatment plans
- Provide therapy focused on daily living activities, cognitive skills, upper extremity function, and adaptive techniques
- Recommend equipment and environmental modifications to improve safety and independence
- Work closely with rehab staff, nursing, physicians, and families to coordinate care
- Monitor resident outcomes and maintain accurate documentation
