About The Position
Human Touch Home Health is seeking a compassionate and skilled Occupational Therapist (OT) to join our growing team serving patients throughout Fairfax County, Loudoun County, and Prince William County. This PRN opportunity offers flexibility, autonomy, and the ability to focus on meaningful patient outcomes. As a Home Health Occupational Therapist, you will work with patients in their homes to help them regain skills for everyday activities, improve function, adapt to physical limitations, and maintain independence. The role emphasizes meaningful patient care and making each visit count, without the constraints of crowded clinics or overlapping appointments.
Requirements
- Active Virginia Occupational Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Communication and Documentation Skills
- Commitment to Delivering Exceptional Patient-Centered Care
Nice To Haves
- Home Health Experience Preferred but Not Required
Responsibilities
- Complete occupational therapy evaluations, reassessments, and discharge visits
- Develop individualized treatment plans based on patient goals
- Provide skilled occupational therapy services in patients' homes
- Assess home safety and recommend adaptive equipment and modifications
- Educate patients and caregivers on techniques that promote independence and safety
- Collaborate with physicians and interdisciplinary team members
- Maintain timely and accurate documentation
