About The Position

Amazing Care Home Health Services is seeking an Occupational Therapist (OT) to provide in-home, individualized therapy to adult clients in the Denver Metro area, CO. The OT will help patients improve independence, mobility, and overall quality of life through skilled, client-centered care. The role involves developing and implementing personalized treatment plans, conducting evaluations, providing therapy focused on ADLs, mobility, safety, and independence, educating patients and caregivers, maintaining documentation, and collaborating with a multidisciplinary care team.

Requirements

A valid Occupational Therapist license in Colorado
Proven experience in home health or a related field
Excellent communication and interpersonal skills
Proficiency in electronic medical record documentation
A flexible and adaptive approach to meet the diverse needs of your clients

Responsibilities

Develop and implement personalized treatment plans
Conduct evaluations using standardized tools and clinical expertise
Provide therapy focused on ADLs, mobility, safety, and independence
Educate patients and caregivers on home programs and adaptive equipment
Maintain timely and accurate clinical documentation
Collaborate with a multidisciplinary care team to ensure coordinated support
