About The Position

We are seeking a dedicated and compassionate Occupational Therapist (OT) to join our team. In this role, you will provide home-based occupational therapy services that help patients regain independence and improve their ability to perform daily activities safely and confidently in their own homes. Requirements

• Current, active Occupational Therapist license in the state of Pennsylvania (required).
• Strong clinical assessment and problem-solving skills.
• Compassionate, patient-centered approach.
• Excellent communication, organization, and documentation abilities.
• Valid driver’s license, reliable transportation, and current auto insurance.

Nice To Haves

• Previous home health or hospice experience preferred.

Responsibilities

• Perform comprehensive occupational therapy evaluations and develop individualized care plans.
• Deliver skilled occupational therapy interventions to home health and hospice patients.
• Instruct patients and caregivers on therapeutic exercises, adaptive equipment, safety, and techniques to improve independence in activities of daily living (ADLs).
• Collaborate with physicians, nurses, and the interdisciplinary team to ensure coordinated care.
• Monitor patient progress, adjust treatment plans as needed, and document all care provided.
• Educate patients and families on home safety modifications and techniques to prevent falls and injuries.
• Travel to patient homes within the Lehigh Valley and surrounding areas.
