About The Position
FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one care to older adults in Boynton Beach, Florida, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians may also provide telehealth and other virtual care when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and support patients’ mobility, independence, wellness, and quality of life. Occupational Therapists work in a collaborative, interdisciplinary environment and are supported by clinician-informed leadership and a culture focused on evidence-based care, professional growth, and clinician-driven decision-making. FOX describes full-time, part-time, and PRN/Fox Flex employment pathways. Full-time roles are salaried with benefits; part-time benefits eligibility depends on minimum hours, and PRN/Fox Flex compensation is per unit. New graduates are eligible for FOX’s Emerging Professionals Mentor Program. The position’s physical demands include lifting, performing CPR, assisting with transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, carrying out therapeutic activities, driving to care locations, and wearing PPE when required.
Requirements
- Valid Occupational Therapy license in the state or states of practice, or eligibility to obtain one.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; providing moderate to maximum assistance with transfers and bed mobility; repositioning patients; standing and walking for extended periods; climbing stairs; and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities and wear PPE when required.

Responsibilities
- Provide one-on-one occupational therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy sessions that support patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
