JTC Home Health Inc. is looking for a talented and motivated Part-Time Occupational Therapist (OT) to join our team serving patients in the beautiful San Diego area and neighboring North County San Diego communities. If you excel at helping patients regain independence in their daily lives, we want you! Requirements
- Current, active California Occupational Therapist (OT) License.
- Valid Driver’s License and reliable, insured personal transportation for community visits.
- Strong skills in functional assessment, patient motivation, and goal setting.
- Current CPR/BLS Certification.
Nice To Haves
- Experience in home health or outpatient rehabilitation is highly preferred.
Responsibilities
- Conducting comprehensive evaluations to assess patient function, cognitive skills, and ADL/IADL performance.
- Developing, implementing, and managing individualized, goal-oriented Occupational Therapy treatment plans.
- Focusing on activities of daily living (dressing, bathing, cooking, etc.) to maximize patient independence and safety at home.
- Collaborating with RNs, PTs, and Speech Therapists to ensure coordinated, high-quality care.
- Maintaining professional and timely documentation as required by Medicare and state guidelines.
