About The Position
We are seeking a dedicated and experienced Occupational Therapist (OT) to join our home health team. The ideal candidate will possess strong clinical skills, effective communication abilities, and a passion for providing quality patient care.
Requirements
- Minimum of 2 years of experience in a healthcare setting, specifically in home health.
- Current California Occupational Therapist license required.
- Basic Life Support (BLS) certification required.
- A valid Driver’s License and proof of automobile insurance are necessary.
- National Provider Identifier (NPI) number required.
Nice To Haves
- COTA with home health experience will be considered if no OT applicants are available.
Responsibilities
- Conduct evaluations and develop individualized treatment plans for patients.
- Implement therapeutic interventions to enhance patient functional abilities.
- Monitor patient progress and adjust treatment plans as necessary.
- Provide education and support to patients and their families.
- Document patient care and progress in compliance with regulatory standards.
- Collaborate with multidisciplinary teams to deliver comprehensive patient care.
