About The Position

The Bayshore Integrated Care Solutions (ICS) program focuses on transforming the transition from hospital to home for older adults across Ontario through interdisciplinary care teams and bundled care pathways. As an Occupational Therapist with ICS, you will be an integral part of a collaborative, integrated care model, working within one interdisciplinary team and using a shared client record. Your role will involve assessing, treating, educating, and managing clients to help them maximize independence, safely transition home, and achieve meaningful outcomes in their communities. This position offers the opportunity to see the direct impact of your interventions throughout the client's recovery journey.

Requirements

  • Master's Degree in Occupational Therapy or recognized equivalent.
  • Registered and in good standing with the College of Occupational Therapists of Ontario (COTO).
  • Passion for supporting older adults and helping clients achieve successful transitions home.
  • Excellent communication, collaboration, and relationship-building skills.
  • Strong clinical reasoning and decision-making abilities.
  • Ability to work independently while thriving within an interdisciplinary team environment.
  • Demonstrated proficiency with electronic documentation systems and secure digital communication.
  • Commitment to lifelong learning and evidence-informed practice.
  • Valid Ontario driver's license and access to reliable transportation.

Responsibilities

  • Complete comprehensive occupational therapy assessments and develop individualized treatment plans, goals, and discharge recommendations.
  • Conduct ongoing evaluation and modification of treatment plans based on client progress and changing needs.
  • Support clients transitioning from hospital to home through evidence-based interventions that promote safety, independence, and quality of life.
  • Collaborate with interdisciplinary team members to ensure coordinated and seamless care delivery.
  • Communicate assessment findings, recommendations, and education to clients, families, caregivers, and healthcare partners.
  • Maintain accurate and timely clinical documentation in accordance with the standards of the College of Occupational Therapists of Ontario.
  • Provide clinical guidance and direction to support personnel involved in the delivery of Occupational Therapy services as appropriate.
  • Participate in case conferences, care planning discussions, and collaborative decision-making processes.
