About The Position

H2 Health Prime Living provides 1:1 patient care in senior living communities in the Mandarin area of Jacksonville, FL. As an Occupational Therapist, you will help seniors maintain independence, confidence, and quality of life through meaningful, goal-driven interventions. You will also collaborate with home health partners within the same communities to ensure consistency and seamless transitions for patients and staff.

Requirements

• A graduate of an accredited Occupational therapy program.
• An active or eligible state license to practice.

• Prior experience is preferred, but new graduates are welcome!
