About The Position

Greenfield Rehabilitation Agency is seeking an experienced Occupational Therapist (OTR) to join our exceptional therapy team at a highly respected, comprehensive continuum-of-care senior campus in Prairie Du Chien, Wisconsin. This dynamic SNF-based campus includes short-term rehabilitation, long-term care, assisted living, and independent living. This role is best suited for a clinician who is confident navigating complex patient presentations, fast-paced transitions of care, and interdisciplinary collaboration. You will have the opportunity to practice at the top of your license while delivering high-quality, patient-centered care across multiple levels of care.

Requirements

• Minimum of a Bachelor's Degree in Occupational Therapy.
• State of Wisconsin OT license.
• Strong clinical reasoning and patient identification skills.
• Experience managing medical complexity and changing functional status.
• Comfort collaborating with nursing, therapy, and interdisciplinary teams.
• A commitment to quality-driven care over volume.
