About The Position

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients. This role comprises 100% travel within our service region, specifically to patients' homes in Butte County, Shasta County, and Yuba Sutter counties and surrounding communities. We are hiring for Full-Time (40 hours per week, M-F, 8am-5pm) or Part-Time/Per-Diem (10-20 hours per week) roles, with the possibility of expanded hours in the future.

Requirements

  • One year experience as a home health care provider
  • Current California State license as an Occupational Therapist and stays updated through independent study and continuing education
  • Current CPR certification
  • Valid California Driver’s License
  • Eligible to be insured under our liability policy with a clean DMV report
  • Your own vehicle and proof of current auto insurance
  • COVID vaccinated or have an exemption
  • Demonstrated technical competence and accountable for work output.
  • Demonstrates proficiency in performing occupational therapy procedures and modalities and in assessing the effects of these
  • Demonstrates sensitivity with patient, family, and their medical team
  • Ability to work with others, at all levels within the organization, and collaborate effectively.
  • Above-average interpersonal, problem-solving, and written and oral communication skills.
  • Professional behavior when on duty
  • Ability to maintain patient records and able to organize daily schedule

Responsibilities

  • Evaluate patients upon physician diagnosis
  • Assess patient and identify deficits impacting self-care, functional mobility, behavior, cognition and patient’s ability to function in their life’s role
  • Deliver occupational therapy to patients in their own homes
  • Help patients improve their motor skills, coordination, and balance through therapeutic exercises and activities
  • Assist patients in developing or regaining daily living skills, such as dressing, bathing, and eating
  • Teach patients how to use adaptive equipment and assistive devices to enhance their independence
  • Collaborate with other healthcare professionals, such as physicians, nurses, social workers, physical therapists and speech therapists, to coordinate patient care
  • Monitor and document patients' progress during therapy sessions
