About The Position

Summit Home Care and Hospice is searching for an experienced and motivated Occupational Therapist (OT) to join our Home Health team. At Summit, we believe healthcare should be personal, compassionate, and delivered where it matters most - at home.

Requirements

  • Graduate of an accredited occupational therapy curriculum.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Active license to practice as an occupational therapist in the state of Ohio.
  • Strong verbal and written communication skills, excellent organizational abilities, and the capacity to work independently and as part of a team.

Nice To Haves

  • 2 years of previous clinical experience as an occupational therapist, preferred.
  • Prior home health or community-based experience is preferred but not required.

Responsibilities

  • Conduct comprehensive evaluations of patients’ functional status, including muscle strength, endurance, coordination, communication skills, self-care ability, and work capacity.
  • Assess home environments for safety hazards and barriers to independence; recommend and coordinate necessary adaptive equipment.
  • Collaborate in developing individualized plans of care that align with patient goals and organizational standards.
  • Perform work capacity evaluations for patients planning to return to employment and facilitate referrals for vocational training as needed.
  • Design and implement treatment programs with measurable goals to improve patient function and independence.
  • Communicate care plans and progress updates to referring physicians and interdisciplinary team members.
  • Teach and retrain patients in daily living activities lost due to illness or injury, following organizational guidelines.
  • Fabricate and fit splints and instruct patients in the use of adaptive equipment, prosthetic, and orthotic devices to enhance functional ability.
  • Maintain accurate clinical documentation, including progress notes and reports, and promptly communicate changes in patient condition to the physician.
  • Coordinate care with other healthcare professionals to ensure efficient, patient-centered service delivery.
  • Participate in team meetings, case conferences, and in-service training to support comprehensive care planning.
  • Identify additional patient and caregiver needs and initiate referrals for other home health services as appropriate.
  • Educate families, caregivers, and staff on treatment plans and techniques to promote continuity of care.
  • Supervise Certified Occupational Therapy Assistants (COTAs) in compliance with state regulations and organizational policy.
  • Evaluate home health aide care every two weeks when therapy is the only skilled service provided.
  • Contribute to discharge planning to ensure smooth transitions and continuity of care.
