About The Position

Dove Healthcare is hiring an Occupational Therapist at its Fennimore location. The OT plans, develops, organizes, implements, evaluates, and directs occupational therapy and related programs and activities in accordance with rules, regulations, and guidelines governing a skilled nursing facility. The position participates in discharge planning, patient assessments, and the development and implementation of patient care plans, including quarterly reviews. The OT maintains treatment records, patient files, and progress notes as required. New graduates are encouraged to apply.

Requirements

• Master's degree in occupational therapy from an accredited college or university, or a bachelor's degree for those grandfathered in.
• Current occupational therapist license in the State of Wisconsin.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct occupational therapy programs and activities in accordance with rules, regulations, and guidelines governing a skilled nursing facility.
• Participate in discharge planning, patient assessments, and the development and implementation of patient care plans as necessary.
• Maintain treatment records, patient files, and progress notes as required.
• Participate in patient assessments and care plans, including quarterly reviews.
