We are seeking a compassionate and motivated Occupational Therapist (OT) to join our PRN rehabilitation team in a Skilled Nursing Facility (SNF) setting. Position Summary The Occupational Therapist evaluates and treats patients with physical, cognitive, functional, and mobility limitations. The OT develops individualized treatment plans, collaborates with interdisciplinary teams, supervises Occupational Therapy Assistants (OTAs), and supports patients in achieving their highest level of independence with activities of daily living (ADLs).

Requirements

  • Bachelor's, Master's, or Doctorate degree in Occupational Therapy.
  • Current Occupational Therapist license in the state of practice.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Strong communication, documentation, and clinical decision-making skills.

Nice To Haves

  • Experience in Skilled Nursing Facilities (SNF), post-acute care, rehabilitation, long-term care, or geriatrics.
  • Experience supervising Occupational Therapy Assistants (OTAs).

Responsibilities

  • Perform occupational therapy evaluations within 24-48 hours of physician orders.
  • Develop and implement individualized treatment plans.
  • Provide skilled occupational therapy services in accordance with physician orders and established plans of care.
  • Evaluate and treat deficits related to: Activities of Daily Living (ADLs), Functional mobility, Cognition, Strength and endurance, Positioning and safety.
  • Supervise Occupational Therapy Assistants (OTAs) in accordance with state practice acts and company policy.
  • Collaborate with physicians, nursing staff, patients, families, and interdisciplinary team members.
  • Participate in care plan meetings and discharge planning.
  • Complete accurate documentation, progress notes, evaluations, discharge summaries, and daily billing.
  • Educate patients, caregivers, and facility staff on therapy goals and maintenance programs.
  • Recommend adaptive equipment and environmental modifications to enhance patient outcomes.
  • Support student clinical education programs as applicable.
  • Maintain compliance with AOTA standards, state licensure requirements, and company policies.
