About The Position
Constellation Home Care is seeking a per diem Occupational Therapist to join its home care team in Norwalk, Connecticut. This flexible-hours, field-based position involves helping people with physical, mental, or developmental conditions develop, recover, or maintain the daily living and work skills they need to perform everyday activities confidently and independently. Newly licensed occupational therapists are encouraged to apply. The Occupational Therapist provides client-centered care in patients’ homes, evaluates functional status and home environments, identifies hazards, barriers, and equipment needs, and educates patients and caregivers. The role includes helping develop individualized plans of care and completing clinical documentation using the electronic medical record system. Requirements
- Licensed Occupational Therapist in the state of employment.
- At least 1–2 years of clinical experience.
- Reliable transportation and willingness to commute to provide in-home patient care.
- Basic knowledge of the iOS operating system.
- Willingness to participate in the company’s in-house and field orientation training.
Nice To Haves
- Home care experience preferred.
- OASIS experience strongly desired.
Responsibilities
- Provide occupational therapy to patients by assessing and understanding their care plans.
- Evaluate patients’ functional status and home environments for hazards or barriers to independent living, and identify equipment needs.
- Educate patients and caregivers about disease processes, medications, plans of care, and individualized treatment plans.
- Participate in developing individualized plans of care.
- Maintain and process documentation in compliance with agency standards using the electronic medical record system.
- Submit clinical documentation, time records, and other information within required time frames.
