Requirements
- Current California Occupational Therapist (OT) License required
- Graduate of an accredited Occupational Therapy program
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal abilities
- Ability to work effectively within an interdisciplinary healthcare team
Nice To Haves
- Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care experience preferred
- Commitment to delivering compassionate, patient-centered care
Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide occupational therapy services to improve functional independence and quality of life
- Assess activities of daily living (ADLs), mobility, cognition, and safety needs
- Implement evidence-based therapeutic interventions
- Collaborate with physicians, nursing staff, therapy team members, and families
- Educate patients and caregivers regarding treatment goals and progress
- Participate in care planning meetings and interdisciplinary team conferences
- Recommend adaptive equipment and environmental modifications as appropriate
- Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines
- Monitor patient progress and modify treatment plans as needed
