About The Position

Stern at Home Therapy is hiring a part-time Occupational Therapist to evaluate and treat patients in their homes across the Poughkeepsie, New York area. The role provides one-on-one care, with flexible scheduling and caseloads managed around the therapist’s availability and patient needs. The therapist has clinical autonomy over evaluations and individualized treatment plans and receives administrative support for scheduling, coordination, and patient logistics. Shadowing is available for occupational therapists who are newer to home health.

Requirements

• Active Occupational Therapist license in New York.
• Reliable transportation.
• Strong clinical, communication, and time-management skills.
• Ability to work independently in a home-based setting.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient's functional needs and goals.
• Help patients improve independence, safety, mobility, and daily function.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, and members of the clinical team.
• Coordinate with the administrative team regarding scheduling, patient care, and logistics.
• Manage a caseload based on availability and patient needs.
