About The Position

Care Central Professionals Home Health is seeking a dedicated Occupational Therapist (OT) to join our compassionate and patient-focused team in Roseville, CA. Requirements

Current California OT license in good standing.
Graduate of an accredited Occupational Therapy program.
At least 1 year of clinical experience (home health preferred).
Excellent communication and organizational skills.
Valid driver’s license and reliable transportation.
Minimum of 1 year of clinical experience (home health experience preferred).
Strong communication and documentation skills.
Ability to work independently and manage time effectively.

Responsibilities

Conduct skilled OT evaluations and develop individualized care plans.
Provide interventions to improve safety, independence, and quality of life.
Educate patients, caregivers, and families on adaptive strategies, equipment use, and home safety.
Collaborate with our interdisciplinary team to ensure holistic, patient-centered care.
Maintain accurate, timely, and compliant documentation.
Conduct skilled occupational therapy evaluations and develop individualized treatment plans.
Provide therapeutic interventions to improve patients’ independence in daily living activities.
Educate patients, caregivers, and families on adaptive techniques, home safety, and use of equipment.
Collaborate with interdisciplinary team members to ensure comprehensive care.
Document all care provided in compliance with agency, state, and federal guidelines.
Participate in care conferences and contribute to ongoing care planning.
