Requirements
- Current California Occupational Therapist (OT) License required
- Strong clinical assessment and treatment skills
- Excellent communication, organization, and interpersonal abilities
- Compassionate, patient-centered approach to care
- Ability to work effectively within an interdisciplinary healthcare team
Nice To Haves
- Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care experience preferred

Responsibilities
- Evaluate residents and develop individualized treatment plans based on functional needs and goals
- Provide occupational therapy services to improve independence with activities of daily living (ADLs)
- Implement evidence-based therapeutic interventions and adaptive strategies
- Assess cognitive, physical, and functional abilities to maximize patient outcomes
- Collaborate with physicians, nurses, therapists, and interdisciplinary team members
- Educate residents, families, and caregivers on treatment plans, progress, and home exercise programs
- Participate in care planning meetings and interdisciplinary team conferences
- Recommend adaptive equipment and environmental modifications when appropriate
- Maintain accurate and timely documentation in compliance with facility, state, and federal regulations
- Monitor patient progress and modify treatment plans as needed
