Senior LIFE is an innovative home and community based Medicare and Medicaid funded program which provides all-inclusive healthcare services and support to seniors living in the community. This position requires an OT with a strong background as well as a desire to provide meaningful service to a diverse elderly population. The OT will be a central figure in expanding and enhancing Senior LIFE’s mission and; therefore, candidates should be versatile, creative critical thinkers, who can thrive in a team-first environment. Topnotch communication and managerial skills are also vital to ensuring long-term success in this position.

Requirements

• Licensed occupational therapists in Pennsylvania
• Committed to participating in the necessary classes/workshops to retain his/her license
• At least one year of experience as an OT, specifically working with an elderly population
• Demonstrate clinical competency as such
• Topnotch communication and managerial skills

• Additional management work in rehabilitation or a related medical/social field

Responsibilities

• Developing occupational treatment plans for all new and existing members, beginning with comprehensive assessments and evaluations and in-depth reviews of overarching Plans of Care
• Documenting all visits, care activities and changes to member status, to ensure cohesiveness across various departments and interdisciplinary personnel
• Carrying out periodic assessments to best enhance and expand care plans
• Evaluating members’ home environments to minimize hazards, and provide additional in-home support, as needed
• Educating LIFE Center staff and member families to ensure OT treatment plans are executed when members are in and out of the center
• Collaborating with interdisciplinary teams and keep all participants informed as to pertinent OT changes as they relate to members’ medical, social or emotional care
• Working in tandem with the dietary staff to develop adaptive feeding techniques and best practices for members, as needed
