The Alex is a community-based, not-for-profit organization that has been providing integrated primary health care, community support services, and housing-focused programs since 1973. They serve individuals and families facing barriers to traditional health, housing, and social service systems. The Pathways Assertive Outreach (PAO) program is an evidence-based Assertive Community Treatment team that operates within the ACT Fidelity framework and is guided by Recovery and Assertive Engagement principles. Services are delivered by a transdisciplinary team, prioritizing continuity of care for individuals with a chronic primary diagnosis of a psychotic illness. The Occupational Therapist is a key member of the Pathways Assertive Outreach Team (PAOT), reporting to the Clinical Team Lead. This role supports program participants in improving, regaining, and developing skills for daily life, helping them restore or increase their ability to care for themselves and engage in work, school, or leisure activities in an outreach setting (participant homes). The position also involves responsiveness to ACT Team duties, including on-call responsibilities. This is a full-time contract position for 13 months, ending September 3, 2027.
