About The Position

Human Touch Home Health is seeking a compassionate and skilled Occupational Therapist (OT) to join our growing team serving patients throughout Northern Virginia. This is a PRN opportunity offering flexibility, professional independence, and the ability to make a meaningful difference every day. As a Home Health Occupational Therapist, you'll help patients regain the skills, strength, and confidence they need to perform everyday activities safely while remaining in the comfort of their own homes. You'll have the autonomy to create individualized treatment plans, build meaningful relationships, and celebrate every milestone with your patients. The role emphasizes personalized therapy, meaningful connections, and life-changing outcomes, distinct from crowded therapy gyms or rushed appointments.

Requirements

Active Virginia Occupational Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Communication, and Documentation Skills

Nice To Haves

Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky Home Health is a plus
Passion for Delivering Exceptional Patient-Centered Care

Responsibilities

Complete occupational therapy evaluations, reassessments, and discharge visits
Develop individualized treatment plans based on patient goals and functional needs
Provide skilled occupational therapy services in patients' homes
Assess home environments and recommend adaptive equipment and safety modifications
Educate patients and caregivers on therapeutic techniques, energy conservation, home safety, adaptive equipment, and activities of daily living
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
