As a Home Health Occupational Therapist, you’ll provide skilled, one‑on‑one therapy services in the comfort of patients’ homes, helping them regain independence with daily activities and safely navigate their environments. You’ll assess patient needs, develop individualized treatment plans, deliver evidence‑based interventions and educate patients and caregivers to support functional independence and quality of life at home. Whether you’re experienced in home health or transitioning from hospital, outpatient, acute care or skilled nursing settings, this role offers the opportunity to practice at the top of your license with greater flexibility, autonomy and meaningful 1:1 patient relationships.
