About The Position
Human Touch Home Health is seeking a licensed Occupational Therapist to provide home health services on a per diem (PRN) basis throughout Fairfax County, Loudoun County, and Prince William County, Virginia. This role offers flexibility, autonomy, and the opportunity to provide one-on-one care that helps patients regain independence in their daily lives.
Requirements
- Active Virginia Occupational Therapist license
- Valid driver’s license and reliable transportation
- Strong documentation and communication skills
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete occupational therapy evaluations, reassessments, and discharge planning
- Develop treatment plans to improve activities of daily living and functional independence
- Provide skilled therapy services in patients’ homes
- Educate patients and caregivers on adaptive techniques and safety
- Coordinate care with physicians and the interdisciplinary team
