About The Position
We are seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team. In this role, you’ll have the opportunity to deliver meaningful, one-on-one care that helps patients restore independence, improve function, and return to daily life with confidence. If you are passionate about patient-centered care and want the freedom to make a real impact, this is the role for you. (MIN 15 VISITS PER WEEK REQUIRED)
Requirements
- Current Occupational Therapist (OT) license in good standing
- Strong clinical judgment and patient assessment skills
- Ability to work independently in a home health setting
- Excellent communication and teamwork skills
- Compassionate, patient-centered approach to care
- Valid Occupational Therapy License
- 1 year of occupational therapy experience preferred
- CPR certification
- Valid Driver’s License & Auto Insurance
- HomeCare HomeBase experience preferred
Nice To Haves
- Experience in home health is a plus, but not required
Responsibilities
- Conduct comprehensive clinical evaluations to determine patient needs and therapy appropriateness
- Develop, implement, and update individualized Plans of Care
- Provide skilled OT interventions to improve: Strength, coordination, and range of motion, Functional mobility and safety, ADL performance (self-care, dressing, bathing, eating, etc.)
- Assess cognitive, sensory-motor, psychosocial, and environmental factors affecting function
- Fabricate and maintain splints and adaptive equipment
- Recommend home/environmental modifications to promote safety and independence
- Participate in interdisciplinary care planning (IDG) to ensure coordinated patient outcomes
- Maintain accurate, timely clinical documentation (evaluations, progress notes, orders)
- Follow infection control and safety standards in all patient care activities
