About The Position

Lutheran Home is seeking an Occupational Therapist to join their team. The Occupational Therapist will work with the care team to provide rehabilitation services for individuals in the rehab center and those living in long term care. Interested candidates should be motivated, goal-oriented and dedicated to the mission and "4 C's." This is a PRN opening with a rate of $45. The role will include one mandatory Saturday per month with additional weekday hours available to pick up.

Requirements

Master's degree in Occupational Therapy
Active state licensure required
Familiarity with Microsoft Office

Prior experience working with the adult and/or aging population preferred
Ability to develop and nurture professional and educational relationships with peers and clients

Responsibilities

Conduct and document initial patient evaluation
Contribute to care planning, setting objective and measurable goals
Applying appropriate evidence-based interventions in line with care plan and goals
Evaluate patient response to treatment and report to interdisciplinary team
Document ongoing treatment, make recommendations for contented treatment as necessary
Participate in team meetings, contribute to process/quality improvement
