About The Position

BAYADA Home Health Care is seeking a compassionate and dedicated Occupational Therapist (OT) to join our Hudson County Visits team. This position offers the flexibility of full-time or part-time employment while providing one-on-one care to adult and geriatric clients in the comfort of their homes throughout Hudson County. As a Home Health Occupational Therapist, you will be part of a multidisciplinary team committed to helping clients maintain independence, safety, and quality of life while receiving care where they are most comfortable—at home.

Requirements

  • Current New Jersey Occupational Therapist license.
  • Minimum of one year of recent experience as an Occupational Therapist.
  • Graduate of an accredited Occupational Therapy program.
  • Strong clinical, organizational, communication, and problem-solving skills.
  • Ability to work independently and manage time effectively.

Nice To Haves

  • Prior home health experience preferred but not required.
  • Experience with EMR documentation preferred.

Responsibilities

  • Perform home visits within designated geographic territories.
  • Complete comprehensive evaluations of patients' functional abilities, including ADLs, IADLs, mobility, cognition, communication, and safety.
  • Develop individualized treatment plans designed to maximize independence and functional outcomes.
  • Assess home environments and recommend adaptive equipment and modifications.
  • Collaborate with nursing and other disciplines to develop and implement patient-centered care plans.
  • Educate patients, caregivers, and family members on therapeutic interventions and strategies to improve safety and independence.
  • Accurately document patient assessments, interventions, and outcomes using electronic medical records.
