Brushy Creek Post Acute is a gorgeous 144-bed skilled nursing facility conveniently located next to the hospital in Greer, South Carolina. Our unique campus features 12 cottage-style homes, each housing 12 residents and complete with its own kitchen, creating a warm and inviting environment. Requirements

  • Bachelor's degree in Occupational Therapy required
  • Current Occupational Therapist license in South Carolina and in good standing
  • Must maintain all required continuing education requirements
  • Must remain in good standing with the applicable State Board
  • Strong clinical, communication, and documentation skills

Nice To Haves

  • Master's or Doctorate degree in Occupational Therapy preferred
  • Experience in skilled nursing, rehabilitation, or long-term care preferred
  • Experience with PointClickCare (PCC) and Casamba preferred

Responsibilities

  • Evaluate residents and develop individualized occupational therapy treatment plans
  • Provide skilled occupational therapy services in accordance with physician orders and resident goals
  • Monitor resident progress and adjust treatment plans as needed
  • Collaborate with physicians, nursing staff, residents, families, and interdisciplinary team members
  • Participate in care plan meetings, rehabilitation conferences, and discharge planning activities
  • Complete accurate and timely documentation in accordance with facility, state, federal, and payer requirements
  • Educate residents, families, and caregivers on treatment plans, adaptive techniques, and maintenance programs
  • Recommend adaptive equipment and interventions to improve resident function and safety
  • Conduct in-service training and education for facility staff as needed
  • Maintain compliance with all facility policies, state practice acts, and regulatory requirements
  • Promote resident safety, independence, and overall well-being
