About The Position

Nightingale Home Healthcare is seeking an Occupational Therapist to provide patients with around-the-clock care and support. This role involves interpreting and evaluating recommendations and orders, assessing patients' capabilities and disabilities, and planning and executing progressive treatment programs. The therapist will coordinate treatment programs with the overall rehabilitation plan.

Requirements

  • Licensed OT.
  • Physical within the last year
  • Current TB results (within last 12 months) and CPR Certification.
  • Valid driver’s license
  • Ability to attend an orientation at our Willowbrook office for 1-3 days

Nice To Haves

  • Minimum of 1 year as an OT experience preferred.
  • OASIS and Wellsky (Kinser) experience strongly preferred.

Responsibilities

  • Provide effective patient care using approved treatment techniques and procedures.
  • Evaluate patient's condition, functioning level, and treatment goals, maintaining current records.
  • Plan, administer, and supervise COTAs and HHAs in accordance with the treatment plan after patient evaluation.
  • Administer appropriate evaluations to assess patient's level of function and develop therapy goals.
  • Develop treatment plans for each patient and perform appropriate individual treatment.
  • Document patient treatment in the medical chart daily.
  • Provide therapy services as outlined in the Plan of Care.
  • Assist patient and family with the development and review of home programs, adaptive equipment, and home setting evaluations.
  • Chart services rendered, note changes in patient's condition/family/home situation, revise care plans, record supervisory visits, evaluate patient care and progress, and prepare Discharge Summaries.
  • Maintain confidentiality of client's situation, records, and services according to HIPPA regulations.
  • Evaluate the effectiveness of therapy services to the individual and family.
  • Submit all patient notes and/or visit records on a timely basis.
  • Obtain orders for paramedical services and submit referrals to the Director of Clinical Services.
  • Coordinate total patient care.
